Job summary

St. Elizabeth Physicians is seeking a Care Management Referral Coordinator to join the Care Management Team. This role manages referrals, coordinates the SEP Grant Funded Transportation Assistance, and supports care management with audits, gaps closure, and social drivers interventions.

The CMRC works to create a positive patient impression, collaborate with providers, and ensure timely, effective care coordination across the care team.

Qualifications

• Minimum: Bachelor’s degree in a related field, and/or Associate's degree with equivalent experience.
• Preferred: Bachelor's degree in Human Services.
• Minimum: 2+ years of experience in a health-related or community resources field.
• Valid driver’s license, reliable transportation, current automobile insurance.
• Working knowledge of care management and social determinants of health.

Responsibilities

• Answer all incoming calls to the Care Management Referral Line and provide appropriate resolution.
• Review and evaluate incoming referrals via chart review and assign appropriately using critical thinking.
• Conduct initial evaluations to determine the appropriate team member to route referrals to.
• Proactively identify and create appropriate referrals.
• Coordinate with family and resources to meet patients' needs and follow up as indicated.
• Schedule, cancel, and reschedule appointments for the Care Management Team.
• Link patients to community resources and monitor progress.
• Educate patients with resources, timelines, and goals for accountability and collaboration.
• Collaborate with patient, family, healthcare team, and community supports to coordinate services.
• Complete face-to-face visits with patients when necessary, in PCP offices or community settings.
• Maintain HIPAA-compliant communication and documentation.
