Job summary

Daymark Recovery Services is seeking a Care Management Extender to support care planning, referrals, and ongoing monitoring for clients with behavioral health needs in North Carolina. Under supervision, you will coordinate with providers, educate clients, and document progress in the medical record.

You will facilitate access to services, help maintain a medical home, and assist with authorization processes while ensuring high-quality, person-centered care.

Qualifications

• High school diploma or equivalent is required.
• Experience in care management or related field preferred.
• Training or certification in peer support or CHW programs is a plus.

Responsibilities

• Provide care management extender duties and linkage to needed services.
• Monitor and follow up with clients and referrals.
• Assist in education for health promotion and care planning.
• Maintain documentation of care activities and outcomes.
