Mercy in Missouri seeks a Care Manager to guide discharge planning from admission to discharge in collaboration with the healthcare team. You will support patients with complex or high-risk psychosocial needs and ensure care aligns with Mercy’s mission and Service Standards.

This role requires nursing graduation, RN license in Missouri, BSN, and 2–3 years in acute care; knowledge of InterQual/Milliman guidelines preferred.

Qualifications

• Graduate of an accredited School of Nursing; RN licensure in MO required.
• BSN degree required; 2–3 years acute care hospital experience preferred.
• Care Management or Utilization Management experience preferred; CPR/BLS required at hire or within 90 days.

Responsabilités

• Facilitates discharge planning from admission to discharge.
• Collaborates with healthcare team to ensure quality patient care and safe transitions.
• Intervenes with patients with complex or high-risk psychosocial needs as assigned.
