Job summary

Mercy Health in Ohio seeks a Manager, Care Management to oversee daily operations of the Care Management Department, coordinating with leadership to standardize practices and optimize patient transitions across the continuum.

You will lead the team, manage resources and participate in budgeting and strategic planning, collaborating with nursing, quality, hospitalists and physician leadership to deliver high-quality, cost-efficient care.

Qualifications

• Bachelor of Science Nursing or Social Work is required.
• Masters in Nursing, Social Work, or Healthcare Administration preferred for BSMH; required for RSFH.
• 3 years of recent acute hospital care management experience (required).

Responsibilities

• Oversee day-to-day operations of the Care Management Department.
• Coordinate with leadership to standardize practices and processes.
• Educate care management staff, physicians, and nursing teams on progression of care and safe transitions.
• Collaborate with ambulatory and post-acute providers to ensure seamless transitions of care.
• Support Utilization Management Committee with actionable data and analytics.
• Develop annual departmental goals and budgets; align with system objectives.
