Job summary

Qualifications include RN or SW licensure, BLS, and ACM/ CCM certifications are preferred. The role emphasizes education, cost management and alignment with value-based care against a budget.

Qualifications

• BLS Basic Life Support – American Heart Association is required.
• RN license in state of practice or SW license; CCM/ ACM certifications preferred.
• Bachelor’s in nursing or social work; master’s preferred for some programs.

Responsibilities

• Oversee day-to-day operations of Care Management team and standardize practices.
• Collaborate with leadership to align with population health and care transitions.
• Lead education for staff, physicians and care teams on progression of care.
• Direct utilization of resources and manage departmental budgeting and cost.
• Monitor outcomes with data tools and drive process improvements.
• Ensure compliance with regulations and value-based care initiatives.
