Job summary

CVS Health is seeking a Care Management Coordinator to drive care management activities across the continuum of care. You will assess, plan, implement, coordinate, monitor and evaluate care for assigned members, using tools to support education and resources.

You will collaborate with interdisciplinary teams, lead case conferences, and develop plans of care with the member and care team to meet goals while navigating barriers and ensuring quality.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, or related field.
• Licensed Behavioral Health Professional in Texas, as applicable.

Responsibilities

• Complete comprehensive evaluation of members and determine a care plan aligned with benefit and services.
• Identify high risk factors and coordinate referrals to appropriate clinical care management.
• Utilize motivational interviewing to maximize member engagement and promote healthy lifestyle changes.
• Educate and empower members to make informed medical and lifestyle decisions.
• Assist members in engaging with providers in healthcare decision-making.
• Travel 10–20% locally to meet members face to face; reliable transportation required.
