Job summary

CVS Health is seeking a Remote RN Case Manager to support members across the United States in a 100% remote role emphasizing care coordination, advocacy, and clinical expertise to simplify health care and improve outcomes.

As a licensed RN, you will conduct telephonic assessments, develop care plans, coordinate referrals, and collaborate with multidisciplinary teams while meeting regulatory and policy requirements.

Qualifications

• Active, unrestricted RN license in your state of residence.
• Ability to obtain licensure in non-compact states as needed (Company paid).
• 3+ years of clinical nursing experience.
• Associate degree in Nursing; BSN preferred.

Responsibilities

• Conduct telephonic case management assessments and care planning.
• Evaluate members' medical, behavioral, and social needs using a holistic approach.
• Develop individualized care plans to improve health outcomes and overall wellness.
• Coordinate care, facilitate referrals, and help members access appropriate resources.
• Identify barriers to care and implement strategies to reduce risks and improve engagement.
• Collaborate with interdisciplinary teams to support members with complex health conditions.
• Ensure compliance with regulatory requirements and company policies.
