Job summary

CVS Health is seeking a Care Manager RN to coordinate care for members with complex health needs. You will assess, develop care plans, and ensure access to needed services while partnering with families and providers.

This remote role supports Medicare/Medicaid populations and requires strong clinical and communication skills. Responsibilities include conducting assessments, coordinating across care settings, and maintaining compliant documentation.

Qualifications

• Active RN license in IL (unrestricted).
• Willing to travel 50–75% in Kane County and surrounding areas.
• 3–5 years clinical nursing experience.
• 2–3 years in case management, discharge planning, care coordination, or home health.
• Strong assessment, critical thinking, and problem-solving skills.
• Excellent communication and organizational skills.
• Ability to work independently in a remote environment.
• Proficiency with Microsoft Office and care management systems.

Responsibilities

• Serve as liaison among members, families, providers, employers, health plans, community organizations, and other stakeholders.
• Coordinate care activities for members with complex, chronic, catastrophic, or acute health conditions across the continuum of care.
• Conduct comprehensive assessments to evaluate medical, functional, psychosocial, and environmental needs.
• Develop, implement, and monitor individualized care plans to improve health outcomes.
• Facilitate access to appropriate healthcare services and community resources.
• Engage with members via telephonic and in-person visits as needed.
• Collaborate with interdisciplinary care teams to ensure coordinated, evidence-based care.
• Document care activities per regulatory requirements and standards.
