Job summary

CVS Health is seeking a full-time Case Manager to conduct care coordination, support, and education using care management resources to achieve optimal member outcomes. You will collaborate with case managers, medical directors, and health programs to implement programs and ensure policy adherence, driving effective care for members.

The role requires a health care/human services degree, strong communication and motivational interviewing skills, and the ability to travel within a 125-mile radius

Qualifications

• Degree in Health Care/Human Services related field.
• Case Management Experience.
• Strong customer service orientation and problem solving skills.
• Excellent communication/telephonic skills.
• Excellent motivational interviewing skills/ability to build rapport telephonically.
• Excellence in documentation and regulatory compliance.
• Highly organized and self-driven; ability to work with minimal supervision.
• Ability to travel within 125 miles round trip for 1 day in the field.

Responsibilities

• Consults with case managers, supervisors, medical directors and health programs to overcome barriers to goals.
• Presents cases at case conferences for multidisciplinary review and optimal outcomes.
• Identifies and escalates quality of care issues.
• Demonstrates negotiation to secure options and services for member benefits and needs.
• Delivers motivational interviewing to maximize member engagement and healthy changes.
• Provides coaching and support to empower members to make independent medical decisions.
• Encourages member participation with providers in decision-making.
• Conducts comprehensive evaluations of referred members’ needs and recommends case resolution.
