Job summary

Qualifications

• High School Diploma required.
• Valid drivers license (USA) required.
• Must be 18 years of age or older.
• Two years of direct experience providing care and navigating Medicaid delivery system, or two years paid experience with Tailored Care Management eligible population.
• Lived experience with I/DD or TBI, or related lived/paid experience pathways.
• Ability to drive and travel as required.
• Ability to maintain required trainings/certifications.
• Knowledge of agency policies, procedures, and service definitions.

Responsibilities

• Support care managers in delivering Tailored Care Management.
• Perform outreach, engagement, and follow-up with people supported.
• Assist individuals with informed choices regarding care, services, and life decisions.
• Provide services and supports focused on safety, self-worth, confidence, community connection, boundaries, planning, self-advocacy, fulfillment, social supports, and communication.
• Document encounters and submit billing documentation.
• Attend staff meetings, case conferences, trainings, and other required meetings.
• Follow agency policies, procedures, service definitions, and supervisor instructions.
