Job summary

Carolina Outreach is seeking a Care Management Extender to support Tailored Care Management through coordinated care for behavioral health and medical needs in Lumberton, NC. The role involves intensive member engagement, linking to medication management, and facilitating services to improve outcomes.

Ideal candidates have lived experience with behavioral health, are Certified Peer Support Specialists, and can navigate Medicaid systems while engaging with diverse, high-need populations.

Qualifications

• High school diploma or equivalent is required.
• Must be 18 years or older.
• Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist.
• Parent/guardian of an individual with a behavioral health condition with Medicaid experience.
• Two years of paid experience performing Extender Functions, including Tailored Care Management exposure.
• Completion of onboarding training; NC DHHS Tailored Care Management training within 60 days of hire.
• No driving infractions in the last 5 years.

Responsibilities

• Conduct regular outreach to members by phone and in person to assess needs, provide support, and monitor progress.
• Connect members and families to healthcare services, housing assistance, food resources, and community programs.
• Assist in developing and updating individualized care plans to support health and wellness goals.
• Coordinate communication among healthcare providers, members, and families to ensure continuity of care and follow-up on referrals.
