Job summary

Jobtailor in Charlotte, NC is seeking a Care Management Support Specialist to assist Tailored Care Management programs and support care managers in delivering services. Qualifications

• High School Diploma required.
• Valid drivers license (USA) required.
• Must be 18 years of age or older.
• Two years of direct experience providing care and navigating Medicaid delivery system, or two years paid experience with Tailored Care Management eligible population.
• Lived experience with I/DD or TBI, or related lived/paid experience pathways.
• Ability to drive and travel as required.
• Ability to maintain required trainings/certifications.
• Knowledge of agency policies, procedures, and service definitions.

Job description

• Support care managers in delivering Tailored Care Management
• Perform outreach, engagement, and follow-up with people supported
• Assist individuals with informed choices regarding care, services, and life decisions
• Provide services and supports focused on safety, self-worth, confidence, community connection, boundaries, planning, self-advocacy, fulfillment, social supports, and communication
• Conduct skill-building activities related to housing, finances, employment readiness, relationships, health-risk management, and chronic health conditions
• Assist with goals in Plans of Care/Individual Support Plans and comprehensive crisis plans
• Coordinate services and appointments, including reminders and transportation arrangements
• Help individuals overcome barriers to primary healthcare, dental, oral, and vision services
• Link individuals to community-based housing and employment services
• Facilitate person-centered relationships promoting self-determination
• Assist with self-help, advocacy, pre-crisis support, and reasonable-accommodation discussions
• Document encounters and contacts and submit billing documentation
• Attend staff meetings, case conferences, trainings, and other required meetings
• Convey observations and key issues to the supervisor
• Assist with staff orientation, mentoring, and training
• Follow agency policies, procedures, service definitions, and supervisor instructions
• Drive and travel as required

Requirements

• High School Diploma (Required)
• Valid driver's license (USA)
• Must be 18 years of age or older
• Direct experience providing care for and navigating the Medicaid delivery system on behalf of an individual; parent/guardian cannot serve as an extender for their family member
• Two years of direct experience providing care for and navigating the Medicaid delivery system, or two years of paid experience performing relevant functions with at least one year of paid experience working directly with the Tailored Care Management eligible population
• Lived experience with an I/DD or a TBI, with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system
• Must meet at least one of the listed lived-experience, parent/guardian, or paid-experience pathways
• Ability to drive and travel as required
• Ability to maintain required agency, state, and federal training certifications
• Knowledge of and compliance with agency policies, procedures, and service definition guidelines
