Qualifications

• 2+ years in behavioral health or human services.
• 2+ years using MS Office Suite (Outlook, Word, Excel).
• Bachelor's degree or higher in behavioral health or related field required.

Responsibilities

• Provide care coordination, support and education for members.
• Collaborate with care management tools to inform case plans.
• Evaluate member needs/eligibility and coordinate care plans.
• Ensure adherence to regulatory and accreditation guidelines.
