The CDC Foundation is seeking a Linkage to Care Coordinator for the Overdose Data to Action program in Northern Nevada. This in-person/hybrid role will educate, outreach, and link individuals with substance use disorders to care, coordinate with partners, and improve access to community resources.

Under general supervision, the coordinator will identify barriers, support care engagement, and work with staff to streamline referral processes while collaborating with diverse stakeholders.

Formación

• Minimum of three (3) years of full-time, professional work experience in linkage to care, care coordination or wrap around community services.
• Bachelor’s degree in Public Health, Social Sciences, or related field.
• Demonstrated knowledge in overdose prevention, substance use disorder treatment, community-based overdose prevention, recovery, and/or drug policy.
• Experience utilizing trauma-informed and strengths-based approaches.
• Highly motivated and organized with the ability to work well in teams and independently.

Responsabilidades

• Provide outreach, referrals, warm hand offs, and care coordination services for individuals with substance use disorder and co-occurring disorders.
• Advise on evidence-based substance use disorder education to individuals, groups, and organizations.
• Develop networks with external stakeholders and service providers to increase access to resources.
• Assist with assessing and managing referral processes to address challenges for patients and providers.
• Coordinate with community partners on access to evidence-based support and programs, including housing, employment, insurance, food, internet, etc.
