Job summary

CrescentCare in New Orleans is seeking an Opioid Response Linkage Navigator to support community-based navigation and opioid response efforts within the Prevention Department. The Navigator will conduct counseling and testing, link patients to mobile unit services, and coordinate with medical providers to ensure access to medical care and essential support services.

The role supports adherence/retention in HIV/STI/HCV services, data management, and QA measures, with opportunities to collaborate

Qualifications

High School Diploma/GED and 5 years' work experience with priority populations at high risk for HIV and Hepatitis C, required or Bachelors and competency with excel/data entry/computer literacy.
Comparable education and experience will be considered.

Responsibilities

Liaise with Prevention team members to ensure newly identified patients are linked to care.
Provide client-centered navigation and support services to priority populations accessing opioid response services.
Conduct HIV/HCV/STI counseling and testing.
Collaborate with other departments to coordinate services to meet client needs.
Track all encounters with patients receiving services.
Assist with adherence and retention to HIV care services and PrEP.
