Job summary

Public Health – Seattle & King County seeks three career service Social Services Professionals (SUD Care Coordinators) to screen for substance use disorders, link patients to MOUD, distribute naloxone, and provide overdose prevention education.

These roles engage participants in custody and during reentry, offering trauma-informed, patient-centered care through screening, assessment, brief intervention, counseling, and discharge planning with a focus on equity and reducing stigma.

Qualifications

• Two years of relevant experience with or in contact with people involved in the criminal legal system.
• Registration as a Substance Use Disorder Professional (WAC 440-22-240) is required.
• Experience coordinating continuity of care and linkage to services in jail or post-release settings.

Responsibilities

• Engage with people who use drugs using harm-reduction, trauma-informed and person-centered approaches.
• Screening, assessment, brief intervention, crisis counseling, and discharge planning.
• Facilitate access to care and coordinate referrals to MOUD and community resources.
• Advocate for participants during custody and reentry, including insurance enrollment and benefits.
• Distribute harm-reduction supplies and provide overdose prevention education.
• Document services in the electronic health record with accuracy and timeliness.
