Job summary

AltaMed Health Services Corp. seeks a Linkage to Care Coordinator II to streamline enrollment into medical care and ancillary services for newly diagnosed HIV and Hepatitis C patients, and for patients transferring care to AltaMed.

The coordinator will help reduce barriers to care by facilitating access to medications, linking clients with Behavioral Health and substance abuse services, and connecting them with case management and prevention counseling.

Qualifications

• Bilingual English/Spanish required.
• Bachelor's degree in healthcare, social work, or a related field.
• Previous experience in healthcare coordination, case management, or social services preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Coordinate enrollment into medical care and ancillary services for new HIV/Hepatitis C patients.
• Facilitate access to medications and pharmacy services.
• Connect clients with Behavioral Health and substance abuse services.
• Link clients with appropriate case management and prevention counseling.
