Job summary

The Gay & Lesbian Community Center of Southern Nevada is seeking an HIV Advocate (Linkage to Care) to lead care coordination, prevention outreach, and peer-supported pathways into treatment for individuals at risk or living with HIV. This role emphasizes compassionate, comprehensive services, crisis support, and collaborative teamwork within our community health programs.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or related field.
• 2–3 years working with marginalized populations and case management.
• Experience in supportive counseling and group facilitation.

Responsibilities

• Oversee intake to assessment to treatment adherence as part of care coordination.
• Coordinate with team to assist individuals seeking medical care and HIV-related services.
• Supervise peer navigators to promote services and provide one-on-one support.
• Coordinate physician appointments and arrange escort/transportation as needed.
• Provide education on HIV prevention, PrEP/PEP, testing, and wellness.
• Lead outreach activities and community education on HIV prevention and treatment.
• Maintain documentation in EHR and coordinate with program reports.
