Job summary

CrescentCare in New Orleans seeks an Opioid Response Linkage Navigator to provide client-centered navigation and linkage to medical care and prevention services. You will work with the Prevention team to test, link, and guide priority populations to HIV/HCV services and essential support, including data collection and QA duties.

The role requires strong communication skills, ability to work in community settings, and willingness to cover non-traditional hours.

Qualifications

• High School Diploma or GED required; Bachelor’s degree considered.
• 5 years’ work experience with priority populations at high risk for HIV and Hepatitis C, or comparable education/experience will be considered.

Responsibilities

• Link patients to care and navigate to medical services and support.
• Conduct HIV/HCV/STI counseling and testing.
• Coordinate with departments to ensure linkage to prevention programs and QA measures.
• Collect data, support reporting, and QA for program deliverables.
