Job summary

FlexStaff is hiring a Care Delivery Manager RN to work in Brooklyn NY Stillwell location. This temp-to-perm role pays $74/hr and is on-site with a schedule of Mon-Fri 8:30am-5:00pm.

Responsibilities include coordinating care plans, leading IDT meetings, and ensuring regulatory compliance to deliver high-quality, patient-centered care. The ideal candidate will have strong community health/homecare background and proven management experience, with nursing credentials (RN/BSN) and potential case

Qualifications

• Degree from an NLN accredited nursing school (RN, BSN required or at least five (5+) years’ experience in health care setting).
• Successful completion of an accredited Nursing Program is required.
• A Baccalaureate degree is highly desired.
• Case Management Certification recommended within two (2) years of the employment.
• Language: Bilingual preferred.

Responsibilities

• Review and ensure development and execution of personalized care plans to improve satisfaction and outcomes.
• Facilitate IDT meetings and other meetings; support Virtual IDT as needed.
• Manager on call after hours; follow-up documentation and initiating grievances as needed.
• Run clinical delivery reports; represent CHN in all IDT.
• Balance staffing and resources to ensure seamless operation.
