FlexStaff is hiring a Care Delivery Manager to support nursing standards of excellence with advanced professional education, delivering high‑quality care. Strong background in community health/homecare with solid management experience preferred, on-site in New York.

Location is Rockaway Parkway (Brooklyn) NY; pay rate is 74.00 hourly with a Mon-Fri schedule. Temp to perm based on performance and business needs. Focus is on IDT facilitation, care plan execution, and on-call management.

Formación

• RN license current and active in New York State.
• BSN preferred or equivalent nursing education.
• Minimum 2 years administrative/management experience in home health.
• Experience with care delivery and interdisciplinary teams encouraged.

Responsabilidades

• Review and ensure development and execution of personalized care plans to improve satisfaction and outcomes.
• Facilitate IDT meetings, SDRs and related attestations; support Virtual IDT as needed.
• On-call manager for non-operational hours; handle follow ups and grievances in a timely manner.
• Run clinical delivery reports and participate in IDT representation.
• Supervise and support CHN involvement in all IDT activities.
