• Case Management Certification recommended within 2 years

Responsibilities

• Review and ensure development and execution of personalized care plans
• Facilitate Interdisciplinary Team (IDT) meetings and SDR processes
• Serve as on-call manager during non-operational hours and initiate grievances as needed
• Run necessary Clinical Delivery reports
• Represent the Community Health Nurse in all IDT meetings
