Job summary

Washington County Mental Health Services Inc seeks Care Delivery Coordinators to organize outpatient services across four clinic sites and telehealth. You will manage access-to-care timelines, schedule templates in MyAvatar, and coordinate cross-team handoffs to keep care on time.

Ideal candidates have a healthcare operations background, strong data quality skills, and familiarity with HIPAA and 42 CFR Part 2. This is a multi-site, full-time role with comprehensive benefits.

Qualifications

• Education: High School Diploma/GED required; Associate or Bachelor’s degree in healthcare administration, business, public health, or a related field (or equivalent experience) preferred.
• Background in healthcare or behavioral health operations, scheduling, or clinic coordination preferred.
• Proficiency in multi-site/multi-discipline scheduling and standard office software (email, spreadsheets, calendars).
• Strong operational problem-solving under time pressure, meticulous data quality and attention to detail, and knowledge of privacy rules (HIPAA and 42 CFR Part 2).
• Clear, assertive communication without direct authority.

Responsibilities

• Access & Pipeline Management: Support access-to-care timelines; monitor referral and intake pipelines; propose operational solutions to leadership.
• Provider Schedule & Template Administration: Build and maintain multi-site schedules and templates in MyAvatar across service lines.
• Schedule Deviation & Cancellation Resolution: Manage daily schedule adjustments, fill cancellations, place holds, approve overbooks, and resolve schedule errors.
• Panel & Reassignment Tracking: Maintain client panel assignments and trackers; execute reassignments; flag uneven loads or risks.
• Cross-Team Workflow & Handoff Coordination: Standardize non-clinical handoff steps; trigger outreach workflows; facilitate daily huddles and weekly meetings.
• Data Integrity & Operational Reporting: Produce routine performance reports and log disruptions; maintain data accuracy.
• Compliance & Operational Execution: Handle PHI per HIPAA and 42 CFR Part 2; rotate site coverage across four locations.
