Job summary

Asian Counseling and Referral Service (ACRS) is seeking a Care Coordinator I who is Vietnamese-speaking for a Snohomish County focus. You will support vulnerable adults with complex health and social needs, coordinating services across medical, behavioral health, and community supports to improve outcomes.

This hybrid, Mon–Fri role offers a competitive benefits package, a pay range of $58,900–$63,700, and on-site parking.

Qualifications

• Bachelor’s degree (social work, behavioral or health sciences preferred).
• Knowledge of health and social services systems.
• Must complete Health Home Care training (12 modules).
• Bilingual or bicultural skills relevant to the job preferred (Vietnamese).

Responsibilities

• Conduct in-person visits monthly to assess and coordinate care.
• Identify resources and manage referrals to maintain eligibility for essential services.
• Support transitions from hospital to home and coordinate with families and caregivers.
• Document and bill activities accurately and timely.
• Engage in program quality improvement and team meetings.
