Project Access Northwest is seeking a full-time, hybrid remote Care Coordinator II. Pay range is $54,000–$56,150.
FLSA status: Non-Exempt
This role serves Washington State residents by providing care coordination, case management, resource navigation, and referral services to address Health-Related Social Needs (HRSN), regardless of Medicaid eligibility. Grounded in Community Health Worker (CHW) and peer-support principles, the position performs Community Health Worker, Care Coordinator, and Navigator functions to help individuals access healthcare, social services, and community resources. This role is based in King and Snohomish Counties, Washington, and is primarily remote and community-based, requiring regular travel throughout the service area to support outreach, engagement, and community partnership activities.

Ability to read and comprehend instructions and write basic correspondence. Strong verbal and written communication skills required. Ability to enter data with a high degree of accuracy. The expected hiring range for this role is $54,000-$56,150 annually.
