Job summary

Ideal candidates will hold a relevant bachelor's degree or have equivalent experience and knowledge of A&NH/PI cultures. This position offers a salary range of $58,900 to $63,700 per year, medical benefits, and a hybrid work environment.

Qualifications

• Bachelor's degree or relevant experience in social services required.
• Strong knowledge of aging process and community service delivery systems.
• Fluency in Vietnamese preferred.

Responsibilities

• Conduct monthly in-person client visits to ensure continuity of care.
• Ensure accurate documentation and billing procedures.
• Outreach to potential clients and assist with enrollment.

Skills

Bilingual or bicultural skills

Knowledge of A&NH/PI cultures

Effective computer navigation

Education

Bachelor’s degree in social work or related field

Certified Medical Assistance with AA degree
