The ideal candidate will have a Master's degree in social work or similar fields, along with bilingual skills. The role includes hybrid working conditions, emphasizing both on-site and remote responsibilities.

Qualifications

• Bilingual or bicultural skills are required.

• Completion of Case Manager training is beneficial.

Responsibilities

• Develop and implement care plans with clients.
• Conduct comprehensive in-home assessments.
• Advocate for clients in various areas like housing and health.
• Facilitate client transitions to care settings.
