Qualifications

• Master’s degree in a human services field required or QMHP eligibility with 3 years of relevant experience.
• Experience providing community support services to individuals with mental illness/substance use disorders.
• QMHP credential options include multiple listed roles as per CSTAR/CPR guidance.

Responsibilities

• Complete comprehensive assessments, treatment plans, quarterly assessments and crisis intervention.
• Use management strategies focused on outcomes, care coordination, and collaboration with internal and external providers.
• Provide training and supervision of staff, guidance, feedback, field mentoring and oversight of services.
• Monitor and review services, interventions and contacts; evaluate effectiveness.
• Identify program issues and collaborate with leadership to develop solutions.
• Support the team with coverage to ensure continuous program operation.
• Adhere to evidence-based practice models and CARF/DMH standards.
• Communicate information clearly and accurately, orally and in writing.
• Ensure program adheres to agency policies and procedures.
• Undertake other duties and special projects as assigned.
