Job summary

WellMed, part of the Optum family, is seeking a Social Worker to assess, plan, coordinate, and oversee services for patients with complex social determinants of health. This role acts as a liaison to care teams and advocates for patients, ensuring CMS guidelines and plan requirements are met.

The position emphasizes collaboration with physicians, families, and providers, and requires licensure maintenance, strong communication, and the ability to travel to multiple locations.

Qualifications

• Bachelor of Social Work with minimum 3 years' experience beyond required years
• Current BLS certification or must obtain within 30 days of hire
• 3+ years social work experience in a health care environment
• Proficient in MS Office Suite to include Word, Excel, Notes, Outlook
• Reliable transportation for daily travel to various locations
• Valid driver's license in state of work

Responsibilities

• Perform comprehensive evaluations and document findings per CMS standards
• Conduct patient assessments in home, clinics, hospital, or SNF as needed
• Coordinate services within patient plan eligibility using community resources
• Collaborate with external/internal teams to improve patient/provider satisfaction
• Maintain licensure, certifications, and annual training
• Perform other duties as assigned
