Job summary

Caretenders, a part of UnitedHealth Group, is seeking a Social Worker to provide medical social services under physician direction within the interdisciplinary team.

The role focuses on understanding social and emotional factors affecting patient health, assisting in developing an integrated Plan of Care, and guiding the team on available community resources. Requires MSW and a valid driver's license.

Qualifications

• Master’s Degree in Social Work from an CSWE-accredited program.
• Valid driver’s license and access to a reliable vehicle or transportation.
• Licensed Social Worker in the state of residence is preferred.

Responsibilities

• Assist patient, family, physician and health care team to understand social factors affecting health.
• Contribute to development of a comprehensive Plan of Care on a daily basis.
• Instruct team on community resources available to assist patients.
