Job summary

CenterWell Home Health is seeking a Medical Social Worker to evaluate and develop personalized care plans addressing emotional and social needs for home health patients. The role includes directing supervision of the Social Worker Assistant and ensuring compliance with company policies and regulations.

The position requires 1+ year of healthcare social work experience, licensure, and a valid driver's license.

Qualifications

• Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
• Social Worker licensure in the state of practice, if required by state law or regulation.
• A valid driver's license, auto insurance, and reliable transportation.
• Proof of current CPR certification.
• Experience with discharge planning needs, and obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility).
• Knowledge of medications and their correct administration.
• Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
• Flexibility in work hours and travel locally.
• Maintain current licensure certifications and meet mandatory continuing education requirements.
• Must read, write and speak fluent English.

Responsibilities

• Assess the patient's social and emotional state as it relates to illness or injury and needs for care.
• Assess relationships of medical and nursing needs in the home setting, financial resources, and community resources.
• Provide actions to obtain available community resources to help resolve issues that may impede recovery.
• Communicate with patients, families, and associates in the organization.
• Provide ongoing assessment of patient and family needs and responses to treatment and coping.
• Participate in interdisciplinary care team care coordination and serve as a resource.
