Job summary

WellMed, part of the Optum family, is hiring a Social Worker to assess, plan, coordinate and oversee services for patients within their health plan. The role Advocates for patients and collaborates with physicians, family, and providers to promote quality outcomes across the care continuum.

Required Bachelor of Social Work with 3+ years in healthcare, BLS or within 30 days, and strong MS Office skills; valid driver’s license and travel to sites are needed.

Qualifications

• Bachelor of Social Work with at least 3 years’ experience beyond entry level.
• Current BLS certification or must obtain within 30 days of hire date
• 3+ years social work experience in a health care environment
• Proficient in MS Office Suite to include Word, Excel, Notes, Outlook
• Reliable transportation for daily travel to various locations
• Have a valid driver’s license within the state of work

Responsibilities

• Perform comprehensive evaluations and document findings in CMS-compliant manner.
• Conduct patient assessments in settings including home, PCP/specialists clinic, hospital, and/or SNF as needed
• Coordinate services within patient plan eligibility utilizing federal, state, and local resources
• Coordinate with external and internal teams to improve patient and provider satisfaction; participate in PI/IDT meetings
• Maintain licensure, certifications, and annual training
• Perform other duties as assigned
