Job summary

CenterWell Home Health is seeking a Medical Social Worker to join our care team. You will evaluate patients’ social and emotional needs and develop personalized plans of care in the home setting, guiding a Social Worker Assistant as appropriate.

The role requires one year of healthcare social work experience, state licensure where required, and a valid driver’s license. You will coordinate with clinicians, families, and community resources to support optimal patient outcomes.

Qualifications

• Minimum of one year of experience as a social worker in a health care setting, home health, or hospice.
• Social Worker licensure in the state of practice, if required by state law or regulation.
• A valid driver's license, auto insurance, and reliable transportation.
• Proof of current CPR certification.
• Experience with discharge planning and obtaining community resources.
• Knowledge of medications and their correct administration.
• Ability to organize tasks, develop action plans, set priorities, and work under pressure.
• Flexibility in work hours and travel locally.
• Maintain current licensure certifications and pursue continuing education.

Responsibilities

• Assess the patient's social and emotional state and their care needs.
• Assess relationships of medical and nursing needs in the home setting, finances, and available resources.
• Facilitate access to community resources to support recovery.
• Communicate with patients, families, and organization staff.
• Provide ongoing assessment of patient and family needs and responses to treatment.
• Collaborate as a member of the interdisciplinary care team.
• Evaluate the Social Work Assistant's performance for compliance with standards.
• Maintain and submit documentation including case conferences and progress notes.
