Job summary

WellMed, part of the Optum family, is seeking a Social Worker for CareCoach Connect at Cameron County to assess and coordinate care for patients with complex needs. You will advocate for patients and work with physicians, families, and providers to promote quality outcomes.

Responsibilities include comprehensive evaluations, coordinating services across settings, ensuring CMS compliance, and supporting performance improvement initiatives.

Qualifications

• Bachelor of Social Work with at least 3 years of social work experience in a health care setting
• Current BLS certification or obtain within 30 days of hire
• Proficient in MS Office including Word, Excel, Notes, Outlook
• Valid driver1s license and reliable transportation

Responsibilities

• Perform comprehensive evaluations and document findings compliant with CMS standards
• Assess patients in home, PCP/specialists clinics, hospital, or SNF as needed
• Coordinate services within patient plan eligibility using federal/state/local resources
• Collaborate with external and internal teams to improve patient and provider satisfaction
• Active participation in Performance Improvement initiatives and IDT meetings
• Maintain licensure/certifications and ongoing training
• Perform other duties as assigned
