Job summary

WellMed, part of the Optum family, seeks a Social Worker to assess, plan, coordinate and oversee services for patients with complex needs to promote quality outcomes across the care continuum. You will liaison with physicians, families, and providers while collaborating in interdisciplinary teams and ensuring compliance with CMS/Medicare guidelines.

This role requires a Bachelor of Social Work with 3+ years in healthcare and active licensure/certification.

Qualifications

• Bachelor of Social Work with a minimum of 3 years of experience beyond the required years of experience
• Current BLS certification or must obtain certification within 30 days of employment hire date
• 3+ years social work experience in a health care environment
• Proficient in MS Office Suite to include Word, Excel, Notes, Outlook, and other departmental specific programs/applications
• Reliable transportation for daily travel to various locations as assigned
• Have a valid drivers license within the state of work

Responsibilities

• Perform comprehensive evaluations and document findings in a concise/comprehensive manner that is compliant with CMS documentation standards
• Performs patient assessments in multiple settings including home, PCP/specialists clinic, hospital, and/or SNF as needed
• Coordinate services within the patient plan eligibility using federal/state/local resources
• Coordinate with external and internal teams to minimize obstacles and increase patient and provider satisfaction
• Active participation in Performance Improvement (PI) or regional strategic initiatives and in IDT meetings
• Maintain licensure, certifications, and annual training; uphold WellMed core values
