Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure their care needs are met. The role involves planning to meet patient needs, managing length of stay, and promoting efficient resource use.

Responsibilities include psychosocial assessment, care progression, interdisciplinary rounds, and collaboration with physicians and the care team to discharge patients smoothly

• Self-directed with ability to work under stress in a fast-paced environment.
• Strong assessment, interview, organizational and problem-solving skills.
• Knowledge of local, state and federal regulations.
• Ability to collaborate with physicians and care team to progress discharge.

Responsibilities

• Assess chronic disease management needs and psychosocial risk factors for discharge planning.
• Coordinate with PAS, financial counselor and UM to determine insurance needs.
• Collaborate with patient, family, physicians and care team for care progression and discharge plans.
• Meet with physicians and care team to ensure timely patient management.
• Manage all aspects of discharge planning for assigned patients.
• Identify and address barriers to discharge and coordinate post-acute referrals.
