Matter Health is seeking a highly organized Care Coordinator in Memphis, TN to support our care teams. You will serve as the centralized administrative hub, tracking patient status changes, referrals, and care gaps while ensuring timely communications among clinicians and support staff.

You will manage daily care team huddles, coordinate referrals and authorizations, conduct telephonic outreach, and maintain HIPAA-compliant documentation in the EHR.

Qualificações

• 2–3 years in a medical office or care coordination setting.
• Experience scheduling appointments or coordinating referrals.
• Knowledge of medical terminology and EHR documentation.
• Familiarity with insurance authorization processes.
• HIPAA compliance and patient confidentiality understanding.
• Strong organizational skills with ability to manage multiple cases.
• Effective communication and ability to relay updates.

Responsabilidades

• Provide administrative coordination for an assigned care team.
• Coordinate daily care team huddles and follow-up tasks.
• Communicate patient status changes to the team.
• Manage referrals, authorizations, and timely follow-up.
