Job summary
Right at Home Southwest Houston is seeking a dedicated Care Coordinator to organize comprehensive client care plans, coordinate with physicians and families, and supervise caregiver schedules in an in-office Rosenberg, TX setting. You will monitor progress, ensure plan adherence, and actively contribute to high-quality outcomes for seniors.
Ideal candidates have 2+ years in care coordination, strong HIPAA knowledge, and excellent communication.
Qualifications
- Minimum 2 years in care coordination or similar role in home care or healthcare.
- Knowledge of HIPAA and state regulations required.
- Excellent communication, organization, and documentation skills.
Responsibilities
- Coordinate holistic care with physicians, families, and caregivers.
- Ensure adherence to care plans and retrain staff as needed.
- Advocate for clients; accompany to appointments and relay updates.
- Recruit and train caregivers with HR on onboarding and protocols.
- Create and adjust caregiver schedules for ~30–40 clients.
- Document assessments and progress in EMR/CRM; generate quality reports.
- Resolve urgent issues; participate in 24/7 on-call rotation.
