Comprehensive Home Health Solutions in Reno, NV is seeking an experienced Client Care Coordinator to join our Home Care team. This role focuses on developing client care plans, coordinating with caregivers, and ensuring high-quality, compassionate care in the home.

The ideal candidate is organized, an excellent communicator, and comfortable coaching caregivers, while traveling to client homes as needed to support care continuity and safety.

Qualifications

• Experience in home care coordination or senior care.
• Excellent communication with clients and families.
• Strong organizational and multitasking abilities.

Responsibilities

• Develop, maintain, and update individualized client care plans.
• Build relationships with clients and families; serve as a resource for questions and changing needs.
• Coordinate with caregivers, Scheduling, Intake, and Lead Coordinator to ensure continuity of care.
• Conduct supervisory visits and regular client/caregiver check-ins.
• Monitor caregiver performance, safety, and adherence to plans.
• Provide onboarding, training, and mentorship to caregivers.
• Assist with admissions, staffing, and scheduling changes.
• Maintain accurate care coordination documentation.
