Job summary

Right at Home South Central Michigan is seeking a Care Coordinator to develop and maintain client care plans, ensuring exceptional in-home experiences for clients and families.

You will coordinate with clients, families, care partners and referral sources to deliver safe, meaningful care and mentor caregivers on delivery of care plans. A high school diploma with two years of business experience and willingness to on-call after hours are required.

Qualifications

• High school graduate or equivalent with two years of business experience.
• Able to work independently, demonstrating sound judgment.
• Be available as required for on-call duty outside of normal office hours.
• Have a valid driver’s license and use of insured automobile.
• Strong computer skills a must.

Responsibilities
