Qualifications

• Bachelor’s degree and 2+ years’ experience or Master’s degree with 1 year experience in family systems, community resources, case management, or related fields.

Responsibilities

• Develop and implement individualized care plans with youth and family.
• Coordinate CFCCP development and conduct CFT meetings monthly or as needed.
• Foster therapeutic relationships and collaborate with families, CFT members, and partners.
• Document care plans, bill for coordination activities, and manage data in the youth’s EHR.
