Job summary

Educational Service Center of Northeast Ohio seeks a wraparound care coordination professional to assess client ecology and develop family-centered care plans under OhioRISE.

You will lead child and family teams, coordinate services across schools, health, and community providers, and document plans and progress.

Ideal candidates have a related degree or significant experience in child behavioral health, strong communication, and ability to work with diverse families.

Qualifications

• Bachelor's degree in social work, sociology, psychology or related human services field.
• In lieu of a college degree, three years of experience in child behavioral health could be considered.
• Minimum of two years' experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or related field.
• Have a background in family systems, community systems, case management, child and family counseling or therapy.
• Be culturally competent to manage complex cases and work with diverse populations.
• Experience with children and families experiencing SED, trauma, or co-occurring disorders.

Responsibilities

• Align practice with the agency mission, trauma-informed Sanctuary Model, and wraparound values.
• Provide structured service planning and care coordination through high fidelity wraparound.
• Conduct OhioRISE care coordination activities for ICC or MCC as applicable.
• Complete initial face-to-face contact and update Ohiocomprehensive CANS assessments.
• Facilitate child and family teams to develop and review care plans.
