Job summary

Stonington Institute in North Stonington, CT, a division of Universal Health Services, seeks a Care Coordinator to support discharge planning and care continuity for adults in substance abuse treatment.

The role collaborates with the treatment team, completes discharge plans, coordinates referrals, and engages with community providers to ensure seamless transitions.

Bachelor’s degree is required; three years in substance abuse/mental health and discharge planning experience are preferred.

Qualifications

• Bachelor’s degree required; three years in substance abuse/mental health preferred.
• Experience in discharge planning or case management required.
• Knowledge of ROIs and referral processes is a plus.

Responsibilities

• Complete a discharge plan for each client admitted to the detox program.
• Assure appropriate ROIs are completed for referral agencies as required.
• Ensure the Step-Down process is maintained to provide continuity of care.
• Work collaboratively with clients and the treatment team to incorporate client needs and preferences into discharge planning.
• Act as a liaison with community providers and respond to inquiries related to discharge planning.
