Responsibilities

• Develop and educate new departmental staff on the department's vision, mission, and goals.
• Prepare and deliver monthly staff meeting agendas and ensure attendance.
• Lead bed board meetings and share information with staff.
• Assess and coordinate Continuing Care planning for patients and families.
• Ensure discharge plans are timely, appropriate, and resource-efficient.
• Review admissions and continued stays to support discharge planning.
• Supervise Continuing Care Coordinator staff and address performance issues.
• Collaborate with physicians, nurses, case managers and social workers.
• Assist with coordinating post-hospital services (home health, SNF, hospice) as needed.
• Engage community resources to facilitate continuity of care and referrals.
