Community Health Network of Connecticut, Inc. is seeking an Inpatient Discharge Care Manager in the Greater New Haven, CT area.

The role focuses on coordinating discharge plans, assessing barriers to safe discharge, and linking members with HUSKY Health and community resources. Ideal candidates hold an active RN license, have 2+ years of direct clinical RN experience, and can collaborate across hospital and outpatient teams to prevent readmissions.

Qualifications

• Active Registered Nurse License
• Two to four years post-secondary schooling
• Associate's degree or Bachelor's degree preferred
• Minimum 2 years direct clinical care experience as an RN

Responsabilités

• Collaborate with hospital, PCP, home health agency, member, and CHNCT ICM to develop discharge plan and address readmissions.
• Meet with inpatient members to assess barriers to safe discharge including physical, cognitive, psychosocial needs and PCP gaps.
• Provide health education and information on HUSKY Health and community resources.
• Communicate gaps in care with hospital care coordinator, attending physician, ICM and PCP.
