Job summary

SummitStone Health Partners in Fort Collins, CO is seeking a Care Coordinator to support individuals transitioning from acute care settings. You will educate, outreach, and connect clients to Medication Assisted Treatment and community-based supports.

The role partners with emergency departments, primary care, crisis response teams, and behavioral health staff to ensure smooth discharge and follow-up. A high school diploma and 1+ year care coordination experience are required; a BA/BS is

Qualifications

• High school diploma or GED required.
• At least one year of care coordination or case management experience in behavioral health or substance use fields.
• Experience with mental illness and substance use disorders.
• Willingness to pursue CAS/CAC III/LAC certifications preferred.

Responsibilities

• Provide care coordination services for clients transitioning from acute care.
• Engage with emergency departments, primary care, crisis teams, and behavioral health partners.
• Outreach clients via referrals or cold outreach.
• Collaborate with internal teams to coordinate post-discharge services.
• Assist clients in navigating healthcare and community resources.
