Job summary

The Hampton-Newport News Community Services Board is seeking a Care Coordinator for the Regional Crisis Stabilization Center. You will coordinate referrals, discharge planning, aftercare, and transitions of care to help individuals move from crisis stabilization to community-based services.

You will work within a multidisciplinary behavioral health team, connect clients with behavioral health, medical, and social resources, and document care plans and outcomes while maintaining client

Qualifications

• Master's degree in Human Services.
• 1 year of experience in service coordination, psychoeducation, and behavioral health skills training.
• Must be registered with the Board of Counseling to practice as a QMHP, or register as a QMHP - Trainee within 30 days of employment.

Responsibilities

• Coordinate referrals, discharge planning, aftercare services, and transitions of care.
• Connect individuals with behavioral health, medical, social, and community-based resources.
• Provide case management, including assistance with benefits and other supportive service applications.
• Develop individualized service plans and maintain timely electronic health record documentation.
• Collaborate with internal teams and community providers to promote continuity of care.
• Review pre-screening evaluations and assist in determining the appropriateness of referrals for crisis stabilization services.
• Provide direct support and crisis intervention while advocating for client needs and maintaining confidentiality.
