Job summary

Mental Health Resource Center is seeking a Care Coordinator to support adults transitioning from higher levels of care to community-based services. You will assess needs, develop person-centered care plans, and conduct outreach to individuals referred from inpatient facilities or community providers.

The role emphasizes coordination across behavioral and physical health, housing, education, and employment, with a focus on uninsured or underinsured clients and high-utilization users.

Qualifications

• Requires a BA in Social Work or related field.
• Minimum of one year of experience in human services or behavioral health.
• Proficiency in MHRC EHR System within three months; proficiency in Microsoft Office and email required.

Responsibilities

• Coordinates services across behavioral health, physical health, housing, education, employment and other systems.
• Builds trust and rapport with individuals and supports.
• Completes screenings, assessments, and intake processes.
• Develops and updates individualized care plans with teams and families.
• Monitors progress and adjusts services as needed.
• Advocates for resources and supports.
• Facilitates warm hand-offs between providers and community partners.
• Maintains documentation of assessments, plans, and progress.
