Job summary

The Care Center, Inc. seeks dedicated staff to support clients with mental health needs transitioning from custody to independent living. You will guide crisis plan execution, assist with daily living activities, meal prep, budgeting, appointments, and transportation while fostering independence and progress toward goals.

Responsibilities include documenting progress, participating in trainings, and ensuring compliance with policies, laws and confidentiality requirements.

Qualifications

• High school diploma or GED; relevant experience considered.
• Ability to understand and communicate information effectively and follow instructions.
• Must obtain required state and federal clearances (CRIM, FBI) and driver’s license.

Responsibilities

• Provide hands-on instruction on crisis plans and resolution.
• Assist clients with Activities of Daily Living and IADLs.
• Support meal prep, budgeting, finances, appointments, and transportation.
• Support skill development and job placement; monitor progress.
• Maintain detailed documentation and attend staff trainings.
