Job summary

Safe Harbor Recovery Center in Portsmouth, Virginia is seeking a Case Manager to support the treatment team by coordinating care across providers and ensuring services align with each client's individualized service plan. Qualifications

• High School Diploma or equivalent required; Bachelor's degree in Social Work, Counseling or related field preferred.
• One or more years' experience in a behavioral setting preferred.
• CPR certification and de-escalation training required.
• On-site at the facility; not remote.

Responsibilities

• Provides case management and coordinates care across providers per ISP.
• Maintains comprehensive clinical records and tracks client progress toward recovery.
• Initiates discharge planning at admission and coordinates post-discharge aftercare.
• Coordinates referrals and ensures services on the ISP are updated monthly.
• Communicates with referral sources and treatment teams to document care within designated timeframes.
